How do I add my baby?

Fax or send a copy of the baby crib card with a letter stating you would like the baby to be added to the case. Please include the case number, mother’s name, Social Security and telephone numbers.

What time and date is my FS interview?

You will be notified by mail of the date and time of your interview. Interviews are conducted daily from 8:00 am – 4:45 PM, Monday – Friday.

How much am I receiving for FS?

Once your case is approved, you will receive a letter notifying you of your FS allotment.

How am I going to receive my FS card?

By mail, after 10 business days. If you did not receive your card after the 10 days, you can call 1-800-SOCIAL2 (1-800-762-4252)

What is a FS re-certification?

An application to renew benefits.

How do I get a replacement card for FS?

My FS card was lost or stolen how do I cancel it?
